Shawn E Long

Shawn E. Long, MD, FAAFP, MBA

Dr. Shawn Long is a Family Physician with over 20 years of patient care experience. Much of that time has been dedicated to caring for the geriatric population and those with chronic medical problems. He has a special interest in the management of care during transitions such as post-hospitalization or discharge from a skilled nursing unit back to home. As a physician, he has spent a great deal of time utilizing home-health services to help his patients stay independent and happier at home.

Dr. Long’s career has included oversights of all health care professionals including Nursing Aids, Medical Assistants, Nurses, Physicians, and Advanced Practice Clinicians with over 7 years of executive leadership for a community health center system. Dr. Shawn Long will provide oversight of the training and development of the care givers, including emphasis on empathetic, compassionate care for a safe and positive environment.

Dr. Shawn Long focuses on building trust between our clients and their care team by facilitating a team-based care model where client’s needs and preferences are addressed in a coordinated effort. Dr. Long’s version of this care model focuses on collaboration and communication among our team, medical providers, our client, and their family. ameriCARE Pittsburgh caregivers play an important role on the care teams of our clients, sharing important information about their condition while providing the safest, best possible in-home care available.

A note from Dr. Shawn Long:

“I’ve witnessed the stress and discomfort that loss of independence can cause for elderly people. Many times, seniors experience significant declines in physical and mental health because they cannot access non-medical support services or have little to no help communicating with their medical care team. I was inspired to start an in-home care business that focuses on facilitating independence while working to maintain the physical and mental health of our clients by bridging the gap between health care services and in-home non-medical care.”

Nayli Long

Nayli Long, PharmD, MBA

Dr. Nayli R. Long has ten years of experience as a registered pharmacist in the state of Pennsylvania, focusing on providing telephonic counseling and creating individual care plans with customized pharmacologic and non-pharmacologic recommendations for Medicare members. Dr Long has an extensive background in Medicare plans, Formulary Management, Medication Therapy Management, and Medication Adherence.

Dr. Nayli Long has also provided thought leadership in management positions, overseeing business strategies, employee engagement, DEI, and human resources. Prior to starting ameriCARE Pittsburgh, Dr. Long spent the last two years of her career overseeing Human Resources and all Diversity, Equity, and Inclusion efforts for her former team. She has a deep understanding of the service industry and the need to create a sense of belonging and psychological safety in the workplace. She has developed programs that place the employee at the center of the decision-making and developed strategies to maximize talent performance while prioritizing their health and wellbeing. Dr. Long uses this experience to seek out, recruit, and support outstanding caregivers throughout the Pittsburgh area to better serve our aging community.

Dr. Long is laser focused on helping ameriCARE Pittsburgh caregivers feel empowered, motivated, and connected to our team, our clients, and the communities we serve.

A note from Dr. Nayli Long

“During my years as a pharmacist, I saw so many patients who didn’t have anyone to care for them, advocate for their medical needs, or even help them maintain their daily routines. I would spend my time after work visiting patients, bringing them their prescriptions, or helping them around the store to get what they needed for their everyday lives. I would spend my weekends doing brown bag events so patients could come by and talk about their medicines with me. Often, I would go to their homes and simply talk to them for an hour or two so they could stay mentally active and have connection and engagement. It was clear the aging community here in Pittsburgh needed better support and we’re here to help fill that gap.”

Albert-Garcia

Albert Garcia, MS

Albert is a health and wellness expert that has been providing programs for individuals, workplaces, and local communities for the last 10 years. After moving to Pittsburgh to receive his Master’s Degree in Health and Rehabilitation Sciences from the University of Pittsburgh, he began working as a full-time personal trainer at Pivot Physical Therapy. Here he focused on post rehab training for individuals 16-92 years old. Once completing his Masters, he was offered the opportunity to be the Head Wellness Guru for the City of Pittsburgh. In this role, he developed and managed the City of Pittsburgh’s Wellness Program for 2,800 city employees and their families. Albert always had a passion for helping the Pittsburgh community so he found an opportunity to fulfill this passion with a non-profit, Common Threads(CT). With CT, he managed nutrition and cooking programs for over 2,000 children and adults in Allegheny County communities that struggled with food and nutrition insecurity.

In addition to his love for helping others live a healthy lifestyle, he strives to be a servant leader in the Pittsburgh community. Albert serves on three non-profit boards that include: Vice President of the Chronic Disease Health Alliance, Chair of New Leaders Council, and President of the Association of Latino Professionals For America.

With ameriCARE, Albert is driven to serve others with the same care and compassion that he has demonstrated with all the individuals, organizations, and communities that he’s served. He is focused on building and empowering a team to provide unmatched, individualized care.
